Sea Change Health Professionals work with NDIS Support Coordinators who arrange the services of a mobile Physiotherapist or Occupational Therapist in the Illawarra.
Contact us to book an appointment and we can discuss a treatment package that will include the initial consultation, follow up appointments and regular assessments. Your plan may or may not include a Therapy Assistant for cost-effectiveness.
During your initial consultation, we will discuss your goals and availability so we can work on a plan that meets your participant’s needs. We will then set up a service agreement that is based on your NDIS budget and requirements.

The process of information gathering, communication before during and after, and the quality and depth of the reporting documents was fantastic. The NDIA have everything they require to ensure that my client can maintain her current 1:1 SDA funding model, and the next plan will be a 12 month one.
